Nel 1986 a Ron Woodroof (Matthew McConaughey), un elettricista del Texas, viene diagnosticato l'Aids e i medici gli prospettano pochi giorni di vita. Frustrato dalla mancanza di terapie mediche ufficiali e non disposto ad accettare una condanna a morte, Woodroof trova un'ancora di salvezza nell'uso di farmaci illegali e alternativi e crea un giro di contrabbando per renderli disponibili ad altri malati di Aids.
